The fees to be paid for approval to supervise physician assistants are to be set by the board as follows:
(a)
An application fee not to exceed fifty dollars ($50) shall be charged to each physician and surgeon applicant.
(b)
An approval fee not to exceed two hundred fifty dollars ($250) shall be charged to each physician and surgeon upon approval of an application to supervise physician assistants.
(c)
A biennial renewal fee not to exceed three hundred dollars ($300) shall be paid for the renewal of an approval.
(d)
The delinquency fee is twenty-five dollars ($25).
(e)
The duplicate approval fee is ten dollars ($10).
(f)
The fee for a letter of endorsement, letter of good standing, or letter of verification of approval shall be ten dollars ($10).
