Tex. Ins. Code Section 4101.052
Application

(a)

An applicant for a license under this chapter must submit to the department an application on a form prescribed and provided by the department, and include as part of or in connection with the application any information that the department reasonably requires, including information about the applicant’s:

(1)

identity;

(2)

personal history;

(3)

experience; and

(4)

business record.

(b)

The application must be accompanied by the fee required by Section 4101.057 (Fees).
